The clutch is operated hydraulically, you probably have a leak somewhere. Best course of action is to take it to a shop and check the lines, if they're good then its the throwout bearing which you have to take the transmission out to replace. If it is that you might as well get a new clutch and pilot bearing.

Sounds like the throw out bearing. Mine broke at 75K. About 10 miles prior, I got a loose gravel sound under the car before it popped and wouldn't engage.

Clutch pedal not returning sounds like a loss of hydraulic pressure. I'd check to see that the level of brake fluid in the master cylinder isn't low. Beyond that, I doubt you're looking at an easy fix. It's probably the slave cylinder or the clutch master cylinder.
