Head tick, blued/scratched cam

Hi there, I've had a bit of a head tick in my car for about 6 months. It use to come and go along with how loud it would be varied greatly day to day. Though just the other day pulling into a parking lot something seemed to let go to the point where the engine stalled. It was hard to restart and when it did it was running rough and sounded horrible. After pulling into a spot and making sure all coils and injectors were working the noise kind of went away and it started to run smooth. After having new poly motor mounts and trans mount put in on the drive home there was a couple of times it would start to run rough, be down on power and sound bad but would then just smooth out. After getting home and having it sit for a couple of hours I went back out and started it up to this.

The noise seemed to be coming from the passenger side near the back where I had replaced the lash adjusters in a previous attempt to stop the tick. So I decided to pull the valve cover again and take a look.

Found two lobes blued with the one on cyl 4 being the worst along with having a noticeable scratch. Didn't have time to look further as I was heading out of town the next day so I reassembled if it needed to be moved. After sitting for six days I decided to see what if it was still there and when I started it I got this

I'm at a bit of loss of what could be causing this problem? Stuck/sticking follower? Lash adjuster not pumping up/down? I'm going to dig back into it a bit deeper when I get a chance.

Thanks for taking the time to have a look and read. Any help would be appreciated.
