Thats the whole setup in the pic, 1 piece. The threaded side screws into your PCV grommet on the valvecover. Then you plug the PCV valve into the rubber grommet on the top of the JLT pre-filter. All the rest of the plumbing remains the same as far as I know. Check the JLT website and look at the pics of it and you'll see what I mean.

That's fine. Some guys use it on the passenger side and JLT provides a 90* fitting for the hose on that side, they probably only include it if you request it.
