From 99 to 2004 is the horsepower the same?

Up from 360 to 380 in 2001 I think somewhere in there.
20 hp diff.

What did they do to the engine to jump up 20 hp?

Yep .... the driveshaft on the newer models have been changed, also, lightened up with aircraft-grade aluminum.
The MAF went from 80mm to 90mm....I think that the intake could be improved on even more by enlarging the upper intake to manifold.....
The concept vehicle has a SuperCooler gadget that is hooked up from the AC unit to the blower along with the more than adequate intercooler unit that comes stock .... cool idea.
...but the best improvement is on the concept engine itself as it is supposedly getting more than the current 2 valves/cylinder.
All stock Lightning 5.4 motors have the 2 valve setup, so adding more valves and better plugs can really help.

I believe that Ford derates the 03-04 L's hp. I made 356rwhp on a Mustang dyno with less than 1k miles on the truck.

I agree on tghe under-rating. Mine made 352rwhp on a Mustang Dyno with about 5k on it...(also made 450+ tq too)...
