lifters turned

has anyone had a lifter actually turn sideways and destroy itsself and the cam before? how can they get out of the t bone bracket? i found this on one cylinder in mine. both lifters chewed up on sides and a huge flat spot. and from looking down you can see the cam is shot.

i dont think it was valve float though. there is enough seat pressure so that that shouldnt happen with this cam only .480lift and only sees 6000rpm

i can get some pics of the lifters tomarro, but the cam wont be out till i pull the motor. and theres no set plan when i start that.
