Please let me know your thoughts as to what's happening here.

It's a McLeod Street Extreme Ceramic Clutch with McLeod Aluminum Flywheel and Ford performance throw-out and pilot bearings. It's a stock clutch cable but I deleted the detent adjuster below the dash to allow the stock cable to be a bit tighter. The clutch fork is actually hitting the bell outing bolts.

So the fork is sandwiched back against the bell housing on a brand new install?

Or is there a gap between TOB and PP?

The install is a couple years old. Had some issues with engagement so I deleted the detent on the stock clutch cable and now it is doing this. I didn't replace the throwout bearing sleeve when I was in there and I think maybe it's wearing away? I should probably put an adjustable cable in it too? Let me know your thoughts.

That is classic TOB chirp. I know there is some opinion on this but I prefer to have some slight preload on the TOB meaning it’s slightly pressed against the PP at all times. I’ve ran mine like that on my cobra, on my 2001 GT, and on my fox body with no premature failure.

I’ve have always ran the MM firewall adjuster and quadrant with their OEM cable so I can adjust it at the firewall.

This is what I personally do and I know there will be some who disagree but I’ve had very good luck with it.

All this is assuming your TOB is good.

Is it possible to bend pressure plate fingers? Doesn't look like even contact on the TOB.

Also think I found......pilot bearing material? See attached.
