I've heard there's quite a few instances every year of this happening, probably mostly in Japan and Asia. People think they know the special way to cut up the fish to remove the poison, but if you are even slightly off you can easily die from poisoning. They advise to never try to prepare it unless you are a trained professional cook.

From the sound of it, it seems like it's a pride thing. You're a badass if you can prepare it properly, but you only get one chance.
